This position will require local travel within the Southern California area performing on-site inpatient concurrent review in the acute hospital and skilled nursing facilities*** Primary Responsibilities: *  Assists in the development of ACO &amp; dual eligible programs to enhance clinical efficiencies:  *  Collects and analyses data to identify centers of excellence based on clinical outcomes, inpatient length of stay with focus on reducing unnecessary admissions and readmissions, palliative care, surgeon and facility specific data, and readmission data.  *  Utilized evidence based medicine to identify any gaps or barriers.  *  Identifies opportunities for increased outpatient vs. inpatient utilization.  * Assists with coordination and follow up of inpatient activities which include but are not limited to:  * Performs daily on-site and or telephonic concurrent review and discharge planning as needed.  * Coordination of care between hospitalists and specialists.  * Discharge planning.  * Post discharge follow up for medication reconciliation, DME and home health services, specialist and or comprehensive clinical appointments and community resources/social services.  * Develop and implements AppleCare Medical Group and ACO Emergency Room follow up program.  * Develops implements and directs ACO Health Education Activities for beneficiaries, providers and staff.  * Assists in the development, implementation, intervention and monitoring of MSSP quality indicators.  * Develops &amp; implement end of life programs such as POLST, palliative care, and hospice with a focus on the improvement of care around end of life.  * Analyzes data to identify population health needs and implements population programs for ACO beneficiaries.  * Provides objective feedback to ACO Network Management regarding provider performance.  * Participates in Utilization Management Committee/Quality Improvement Committee, Primary Care Provider Office Meetings and hospital Joint Operation Meetings. Qualifications: Requirements:  *  Minimum of an Associates degree in Nursing.  *  Active unrestricted California RN license.  *  2 years of acuteinpatient concurrent review experience in a managed care environment.  *  Reliable transportation, valid California Driver&apos;s license and proof of insurance.  *  Must be willing to travel in southern California to conduct on-site inpatient concurrent review in acute hospital and skilled nursing facilities.  *  Must live in a commutable distance of the Buena Park office location. (must report into the office daily).  *  Ability to work effectively with Physicians and interdisciplinary team.  *  Strong analytical and reporting skills.  *  Strong project management skills.  *  Strong written and verbal skills. Preferred Assets: *  1 years of Supervisory or Management experience in acute or managed care environment. *  Bachelors in Nursing/Health Care or related field.  *  Knowledge of Medicare and California Medi Cal regulations and guidelines. *  Knowledge of quality improvement processes. *  Knowledge of inpatient management processes.  *  Knowledge of CMS ACO requirements and quality indicators preferred.
