### Summary
This position is located in the Office of Quality Management reporting to the Director of Accreditation and the primary purpose of the position is the manager and implementer of patient safety policies and activities developed by the National Center for Patient Safety (NCPS). The NCPS policies are found in the Patient Safety Handbook (directive), nation-wide patient safety training materials and computer systems, and applicable regulations of The Joint Commission (TJC). Direct support is provided to the development, maintenance, and operation of the VHA's patient safety process at the facility level. This includes working with all levels of staff within the facility, including top level management, Root Cause Analysis (RCA) team members, RCA team leaders, and RCA recorders. The incumbent also works with, trains, and directs patient safety activities of similar personnel within the facility, such as risk managers and quality managers. The incumbent is responsible for translating the policies, theories, and strategies into action within the complex interrelationships that exist related to patient safety. The incumbent has primary responsibility for training and implementation of the patient safety and RCA process to clinicians, management, and all relevant support personnel within their facilities and associated facilities and operations (e.g., home care).

### Responsibilities
Establishes administration controls aimed at ensuring approved recommendations from completed RCAs are implemented as proposed. Incumbent is responsible for identifying medical center problems which involve new or revised policies or procedures and brings them to the attention of senior medical center management for appropriate action.
Responsible for obtaining timely actions, responses or decisions on request within and to the medical center and ensures they are consistent with medical center policy.
Incumbent leads, directs and assists medical center staff in the operation of the facility's patient safety program.
Responsible for facility-level final analysis, recommends controversial actions to remedy critical problems alleviating any chance that accreditation might be threatened.
Complies with directives in preparing annual and cyclic planning documents. Incumbent is the facility's lead manager and coordinator of its patient safety program, and authoritative recommendations to the MCD regarding possible need for revision of regulations and policies governing planning activities.
Examines programs and recommends expanding, contracting, combining, separating or realigning them to best meet patient health care needs most economically.
Prepares and/or reviews reports involving trend analyses and ensures follow-up in these areas for the facility's patient safety program.
Incumbent is responsible for identifying medical center problems which involve new or revised policies or procedures and brings them to the attention of senior medical center management for appropriate action.
Analyzes complex and critical data, recommends remedial action.
Maintains program requirements/validation criteria for the facility's patient safety program.
Ensures that RCAs are complete, that all technical requirements are met, and all implementation requirements are accurately identified.
Reviews materials with regards to compliance with the standards of the National Center for Patient Safety, external bodies and accrediting agencies.
Develops background and management options available to deal with complex medical center planning and implementation problems related to the facility's patient safety program.
Plays a critical support role in developing and coordinating internal review systems to assure that both clinical and administrative activities are in compliance with agency and accrediting and regulatory requirements.

### Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement.
Time-in-Grade Restriction - Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements. The grade may have been in any occupation but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment.
You may qualify based on your experience as described below:
Specialized Experience GS-11: Applicants must have one (1) year of specialized experience equivalent to at least the next lower grade (GS-09) in the normal line of progression for the occupation in the organization.
OR
Graduate Education: Ph.D. or equivalent doctoral degree OR you must have completed 3 full years of progressively higher level graduate education leading to such a degree or LL.M, if related. (Copies of unofficial transcripts must be submitted with application package)
OR
Combination of Education and Experience: A combination of graduate education and specialized experience. To calculate your percentage of qualifying graduate education, divide your number of semester hours in excess of 18 semester hours by 18 or the number your school uses to represent one year of full time graduate academic study. To calculate your percentage of experience, divide your months of qualifying specialized experience by 12. Add the two percentages. They must equal 100% to qualify using this option. (Copies of unofficial transcripts must be submitted with application package)
Specialized Experience GS-12: Applicants must have one (1) year of specialized experience equivalent to at least the next lower grade (GS-11) in the normal line of progression for the occupation in the organization.
There is no educational substitution for the GS-12 level.
Examples of specialized experience for both grades would typically include, but are not limited to:
Progressively responsible analytical or administrative, or clinical management or supervisory experience in the health care field. This work may have been performed in an operating health care facility or a higher organizational echelon with advisory or directional authority over such facilities. Work must have involved a close working relationship with facility managers and analysis and/or coordination of administrative, clinical, or other service activities, and provided knowledge of the following:
Missions, organizations, programs, and requirements of health care delivery systems;
Regulations and standards of various regulatory and credentialing groups; and
Government-wide, agency, and facility systems and requirements in various administrative areas such as budget, personnel, and procurement.
Preferred Qualifications:
Certification as a Provider of Healthcare Quality (CPHQ)
Prior experience in Quality Management or Systems Redesign
Experience with the VA Patient Safety program, accreditation and/or regulatory compliance
Certification in Lean management (green belt or black belt training)
Strong writing and communication skills
