Clinical Nurse Manager

Responsible for the clinical management of the Lawrence Avenue Primary Care Facility servicing patients in and around CPH in St. Lawrence county New York. Support supervision of all clinical employees and providers in conjunction with Administrative Supervisor and in collaboration with key function leaders at CPH such as Director of Quality, Director of Patient Safety, and Director of Patient Experience. Is responsible for the management of quality clinical standards for employed and contracted physicians, mid-levels (providers) and staff. Reports directly to the Director of PPM and the VP with oversight over PPM, with indirect reporting to the Chief Nursing Officer.

1. Management of Clinical Services

Coordinates with PPM management team and serves as primary medical and staff resource for all clinical concerns. Supervises and assures effective, high quality clinical activities within the medical office home. Cooperates with all providers and clinical staff in performance of all nursing staff functions in compliance with CPH P&Ps, PPM P&Ps, protocols, guidelines for Clinical Standards of Care.

Establishes, implements and monitors all aspects of nursing activities in support of physicians and mid-level providers, nurse practitioners (NPs), and physician assistants (PAs.

Periodically evaluates and provides ongoing improvement of ongoing Provision of Care standards for all outpatient populations, in conjunction with CPH clinical service team. Will aid in tracking quality, satisfactory clinical outcomes.

Monitors meaningful use related to computer/software utilization, and provides nurse training in meaningful use and other software applications.

Scheduling and oversight of clinical staff and assignment of clinical float personnel.

Promotes a culture of safety in the provision of patient care.

Is knowledgeable about all elements of appropriate 1199 SEIU United Healthcare Workers collective bargaining agreements.

Maintains supplies, medical equipment, and medical ordering.

2. Regulatory /Accreditation

Acts as primary resource for achievement and compliance with all accreditation standards of Det Norske Veritas (DNV). Acts as primary resource for achievement and compliance with all CMS, State of New York rules and regulations on the clinical provision of healthcare.

Acts as primary educator for facility staff on all standards in collaboration with PPM leadership team. Provides effective coaching, feedback and developmental action plans to health site leaders and staff for all aspects of DNV. Monitors all sites for compliance with standards and takes corrective actions as needed. Acts as PPM participant on all appropriate CPH committees for DNV, quality, compliance, CMS, etc. Reports PPM compliance with regulatory standards to appropriate CPH departments. Develops actions plans in collaboration with CPH hospital departments as needed.

Develops and maintains appropriate Clinical standards Policies and Procedures (P&Ps), protocols, guidelines, check lists, etc. Performs periodic review and revision of P&Ps. Acts as expert resource in up-to-date clinical standards and education appropriate staff on changes and updates.

Be a resource to all PPM physicians and staff for best practices initiatives, maintenance of confidentiality standards (HIPAA, etc.) Adheres to CPH and PPM Policies and Procedures.

3. Development

Responsible for the assessing, implementing and evaluating strategies for the management of patients around a provision of care plan. Establishes, monitors and analyzes the effectiveness and appropriateness of processes and resources necessary for the care management of patients and patient populations in conjunction with the CPH clinical services team.

4. Financial

Participates with Director on the preparation and management of the operating and capital budgets of Clinical Services. Contributes to the effective management of medical equipment and supplies in a fiscally responsible manner. Participate in financial reporting and budgeting of facility needs.

5. Other

Develop access to appropriate patient education resources and collaborate with clinical leaders and members of the health care team to make resources available to patients. Example: diabetic education, the State of New York Vaccines for Children (VFC) program.

Acts as primary investigator for patient complaints / issues with provider and clinical staff. Maintains open and active communication with PPM Director, appropriate PPM management team members, CPH departments regarding resolution. Acts to resolve issues within a standard of a quality provider / patient relationship, and the provision of patient-centered care.

Provides a professional environment where the customer/patient/provider/staff are valued. Maintains open and active communication with entire PPM management team.

Participates in ad hoc projects, as assigned.

Assists as appropriate with usage of the electronic medical record (EMR) for all clinical staff.

Integrates CPH mission and core values within clinical services responsibilities. Develops plans as necessary to ensure the successful operation of the facility.

Works in collaboration with CPH and PPM departments as warranted.

Required Experience: 3-5 years clinical experience and/or outpatient practice management in a healthcare setting. Skilled in clinical management, organizing, planning, exercising initiative, judgment, problem-solving, decision-making, development and maintaining effective relationships with providers and clinical staff, patients and the public. Effective verbal and written communication skills. Knowledge in electronic health record (EHR / EMR).
