Job Description



General Summary:Under direct supervision, provides clinical support services to patient care staff by performing a variety of duties related to patient care. Acts as a liaison between the physician/APC and patient/family. Supports the activities and necessary functions related to providing high quality professional care in a patient and family-centered fashion that is customer friendly. Develops a good collaborative, professional relationship that enhances each of their respective functions and promotes improved outcomes and clinical efficiencies. Has a basic understanding of behavioral health and physical health and will work closely with the day hospital/outpatient support staff and the WellSpan system of care.





Duties and Responsibilities: 1 Analyzes chart for missing information such as physician signatures. Also, checks to see if authorizations need to be requested by clinician. 2 Measures and records patient height, weight, pulse, BMI, BP, and waist circumference. 3 Attaches prepared forms and documents to the appropriate patients records as directed by the program nurse and/or physician. 4 Documents and records information into the patient record by entering, recording, storing and maintaining information in written or electronic form. 5 Assist the program nurse with coordinating and facilitating closing of a clients medical record in accordance to program policy or as directed by the assigned physician. 6 Responsible for filing and scanning all pertinent information into medical record (i.e., lab reports, progress notes, initial evaluations, etc.) as needed. 7 Ensures the validity of the releases of information.

+ Computer and keyboard skills


+ Knowledge of and ability in the care and handling of patients in all age groups


+ Knowledge of Lippincott, Lexicomp, Systems of Care, eCare, community services and/or other physician support systems


+ Proficiency in Therapeutic Crisis Intervention (TCI) principles of de-escalation




Required Experience



Preferred Certification - Certified Nursing Assistant


1 year of experience in a health-related field with specialized or technical skills/knowledge
