Job Summary:
Keystone Human Services - Intellectual Disabilities provides supports to adults in a residential setting.
Community Support Associate
Act as a role model and advocate; Assess, in conjunction with the county, the individuals'' needs for services and skill development; With feedback from the county and persons supported, develop, monitor and revise Individualized Service Plans (ISPs) to include but not limited to, financial and medication management, socialization, health maintenance, household management, meal preparation, personal hygiene, etc.; Implement the ISP, assisting persons with activities of daily living and providing needed supports to assist individuals to remain in a community setting; Assist individuals with integration into the community, helping them to access community supports; Provide transportation to the bank, for shopping and to meet other daily needs; Attend monthly meetings to discuss the progress of persons supported and the status of the program; Communicate with professional others, completing required forms, reports and documentation as requested or required; Collect and report on outcome data and quality indicators; In partnership with other SLP staff, be available daily to provide assistance to all persons in the program to address immediate concerns; Attend and participate in all required meetings, appointments, trainings, etc.
Minimum Required Qualifications:
Work Location: Sunbury, Shamokin, Milton, Elysburg, and varied locations throughout Northumberland County
- Must be 18 years of age.
- Must possess a valid PA driver''s license and be able to drive
Have access to a privately maintained vehicle; and operate an agency vehicle when required.
- Be physically able to lift and participate in physical activities of daily living.
- Demonstrate ability to communicate both verbally and in writing
Able to read, interpret, and apply written information such as policies, procedures, person centered plans, training and staff development readings.
- Understand, develop and implement learning objectives.
- Possess basic household management skills.
