Home Care Team Supervisor - Full-time - Chronic Care Services -

The Homecare Clinical Supervisor provides leadership and oversight of patient care services and field personnel supporting effective care coordination inclusive of coordinating referrals, ongoing patient care, making patient and personnel assignments, assuring that patient needs are continually assessed, and assuring the development, implementation and updates of individualized patient plans of care. Essential FuntionsProvides leadership and oversight of all patient care services and field personnel as assigned.Coordinates referrals and the provision of patient care; may provide direct patient care as necessary.Responsible for making patient and personnel assignments, assuring patients are seen timely, within regulatory requirements, organizational policy and physician ordered frequency.Assures that patient needs are continually assessed and provision of care is patient-centered and provided with effective inter-disciplinary collaboration and care coordination.Assures effective development, implementation and updates of the individualized plan of care.Responsible to assure effective staffing plans are in place and appropriate levels of staff are scheduled daily.Responsible for departmental personnel matters pertaining to the employment, training, termination and grievance of field personnel including interviews, hires, oversight of orientation and ongoing development of field personnel.Assure all field personnel meet established productivity, documentation and outcome targets.Provides consistent in-field support and supervision of patient care services by providing ongoing performance evaluation and development of field personnel through supervisory field visits.Participates in administrative and patient care on-call rotation as required.Facilitates the education of staff and Quality Assessment Performance Improvement activities in collaboration with the Quality Education Coordinator and departmental leadership.Assures all field personnel effectively complete required training, competencies, onboarding and annual requirements.Assures adherence to federal, state, regulatory and accreditation standards.Reviews documentation of staff on a consistent basis and assures real-time documentation completion at point of care; assures all required documentation is completed appropriately, accurately and timely per policy. Facilitates regularly scheduled Team Meetings to share team and organization outcomes, metrics and targets; sets expectations and supports staff/team in meeting established outcomes/metrics/targets.Keeps abreast of current research and standards of care to enhance and broaden the scope of clinical practice for colleagues and staffProvides exemplary internal and external customer service.

Minimum EducationAssociates Degree in Nursing Minimum Work ExperienceAt least one year of experience in Nursing Required Licenses/CertificationsCurrent Michigan nursing license required.Valid driver's license and dependable vehicle insured as required by the State of Michigan. Required Skills, Knowledge, and AbilitiesAbility to problem-solve with effective verbal and written communication skills is required.Working knowledge of medical coding for Home Care.Possesses strong nursing processes, critical thinking, advanced assessment skills, and knowledge about the care of pediatric, adolescent, adult, and geriatric patients is required.Interpersonal skills to effectively lead the Patient Care Team are required. Preferred QualificationsPrior experience in a supervisory capacity desired.Knowledge of hospital operations, policies and procedures is desired.
