Senior Document Imaging Representative - Louisville, KY

Responsible for processing medical record documents so that they are ready to scan and are properly set up as part of the legal electronic patient record. More specifically, ensures timely records scanning, preparation, validation and quality control.


I _nclude the following. Other duties may be assigned_ .

+ Record Preparation: Checks each record for poor originals and stamps appropriately. Removes all staples, paper clips, sticky notes, rubber bands. Groups like documents in date order; most current on top and in page order. Copies all damaged documents so that they can go through the scanning process without difficulty. Removes interfaced documents from each record. Securely attaches all rhythm strips and other small documents to 8-1/2 X 11 paper for scanning. Removes all photographs, fetal monitors and preps separately and accordingly. Places all records in alphabetical order by patient last name and within patient type. Creates each batch for inpatient, outpatient and ED according to procedure. Assures all charts are prepped in required time frame. Prepares loose reports for scanning according to procedures.

+ Record Scanning: Ensures that all scanned documents are positioned correctly. Identifies at those that are incorrect and fixes those identified. Identifies documents that are of poor quality and marks for rescanning. Indexes documents to correct encounter and document type. Verifies barcoded documents are correctly indexed. Processes batch to validation and quality control.

+ Record Validation and Quality Control: Identifies when it is appropriate to split and merge documents. Correctly inserts or appends pages/documents when and where appropriate. Releases batch and checks application for any images that may have failed and appropriately assign to correct account number and medical record number. Reviews assigned work queues daily and ensures timely processing of all assignments in the queues.

+ Communication: Routinely seeks to understand the needs of individuals and responds accordingly. Maintains confidentiality of all patient information. Consistently ensures all conversations with customers and peers are appropriate, respectful, and professional.

+ Document Imaging Specialist must display teamwork and commitment while performing daily coding duties

+ Must demonstrate initiative and discipline in time management and assignment completion

+ Other duties as assigned based on leadership request(s)

+ Effective written and verbal communication skills

+ Excellent organizational skills for initiation and maintenance of efficient work flow

+ Regular and reliable attendance and time reporting

+ Good visual acuity

+ Able to operate computer keyboard, mouse and other peripherals as appropriate to accomplish document imaging duties

+ Proven ability to perform quality control checks on user input and output, detecting and correcting basic error functions


+ Minimum: High School Diploma, GED or formal educational equivalent

+ Preferred: Two (2) years' experience in health information management or similar environment.


+ Preferred: RHIT or RHIA credential
