Qualifications:
* Master''s degree in social work (MSW) required
* Post-master''s experience working with homeless, mentally ill adult women
* Minimum of three (3) years administrative and supervisory experience
* Excellent problem solving ability with good crisis intervention and strong interpersonal and presentation skills
* Excellent verbal and written communication skills
* Bilingual preferred
* Ability to work on a regular schedule and handle on-call responsibilities
Responsibilities:
* Carry a caseload of at least five (5) cases, meet with clients at least twice a month, create a housing plan
* Manage and monitor utilization of computerized case management system
* Compile data and run monthly reports; provide statistical data to city and state agencies
* Assist with program planning, development, implementation and evaluation
* Provide direct supervision to case managers and housing specialist;direct workload of staff, make hiring and firing recommendations, complete annual performance evaluation, establish personnel accountabilities and training to support staff
* Provide administrative back-up coverage to the Program Director and Program Supervisor as needed
* Supervise MSW interns/students
* Assist and facilitate individualized services to consumers that meet the diverse needs of this population and focus on discharge-planning goals
* Document case notes in database regarding significant and untoward events and contacts with mental health providers, including Intensive Case Managers (ICMs) and Assisted Outpatient Treatments (AOTs)
* Ensure consumer compliance with medication, program and medical appointments, including morning or evening routines, to help consumers meet treatment goals and maintain appointments
* Assist and monitor individuals’ self-administration of medication; count appropriate doses of medication and maintain medication records in accordance with agency policies and procedures
* Reviews crisis assessment of consumers/residents with staff; review nonverbal and verbal crisis intervention techniques to be used with each individual consumer/resident and staff-actions during a crisis situation
* Inform staff of community resources and ensure staff are accompanying consumers on regularly scheduled or emergency visits to medical treatment facilities, social agencies, government offices, housing interviews, appointments or other locations associated with treatment or assistance of the consumer/resident
* Track client program trends, including length of stay at the site
* Track, review and implement a system that tracks program outcomes
* Provide monthly reports to Program Director on all program outcomes
* Coordinate and liaise with onsite providers (nurse and psychiatrist) to ensure appropriate and safe clinical care to consumers/residents
* Attend and conduct case reviews at clinical meeting to ensure the coordination of proper client care
* Maintain, monitor and ensure confidential client records
* Ensure all discharges from program are entered into the database in adherence to city, state and federal regulations
* Perform monthly database case file review and provide written feedback to Program Director
* Collaborate with an interdisciplinary team
* Participate in clinical case conferences and treatment planning meetings
* Cover the duties of a co-administrator or case manager as needed
* Other related duties as assigned by supervisor
