RN - Field Case Manager (Registered Nurse) New York City

Please note this role will cover: Geographic area of NYC and 5 Boroughs, i.e. Manhattan, Yonkers, Bronx, Brooklyn, Queens, Staten Island.



The field case manager is responsible for assessing and analyzing an injured employee to evaluate the medical and vocational needs required to facilitate the patients appropriate and timely return to work. This role acts as a liaison with patient/family, employer, provider(s), insurance companies, and healthcare personnel.



This role will require regular local travel and requires owning a car and a valid drivers license. In this role you will work no holidays or weekends. This is a great opportunity to work independently as a consultant with additional opportunity to earn a monthly and quarterly bonus.



Fundamental Components:

You will interview employee's in their homes, work-sites or physician offices to provide ongoing case management services. You will evaluate employee progress toward desired outcomes through assessment and evaluation. The field case manager communicate both in person and telephonically with employee, medical providers, attorneys, employers and insurance carriers. Prepares all required documentation of case work activities. You will also coordinate referrals to specialists and alternate treatment provides as appropriate. Registered nurse with current state RN license.



BACKGROUND/EXPERIENCE:

Current active and unrestricted NY RN License CCM preferred, not required, but will be expected to obtain CCM certification when requirements met Willingness to travel up to 80% of each work day Familiar with smartphone, applications, Outlook, Windows 7 Strong communication, organizational and interpersonal skills. Previous case management or home care experience preferred

EDUCATION

The highest level of education desired for candidates in this position is a Associate's degree or equivalent experience.



LICENSES AND CERTIFICATIONS

Nursing/Certified Case Manager (CCM) is desired

Nursing/Certified Disability Management Specialist (CDMS) is desired

Nursing/CRRN - Certified Rehabilitation Registered is desired

Nursing/Registered Nurse (RN) is required
