Responsibilities for Care Coordinators include:

-Engaging members telephonically to coordinate services, community resources, and treatment needs
-Delivering a holistic approach to coordinated care based on the enrollee needs
-Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file
-Creating, reviewing and revising Plan of Care plans and focusing on disease management
-Functioning as an advocate for members and decreasing unnecessary hospitalizations
-Collaborating with the member''s PCP to deliver and coordinate necessary services
-Building relationships with members and their families; assisting them with proper health choices
-Ensuring cases are documented in a timely manner
This position requires driving to member place of residence ( home or nursing facility) to conduct member assessments. Significant documentation, data entry and paperwork required. Applicant must have solid computer skills including data entry, word and excel. Must have strong organizational skills be able to mulit-taks and meet frim deadlines.

Qualifications:
Requirements:
-Bachelor''s level degree in Social Work or other related field
-Minimum of 1 year clinical or case management experience
-Minimum of 1 year experience in long-term care, home health, hospice, public health, or assisted living
Additional Assets Preferred:
-Medicare/Medicaid experience
-A background working in geriatric special needs
-A background in behavioral health
-Experience with electronic charting
-Ability to multitask
-Basic level of proficiency in PC-based word processing and database documentation (Word, Excel, Internet, Outlook)
