Navigant is supporting the State of Tennessee Health Care Finance and Administration with statewide implementation of two practice transformation programs for Federally Qualified Health Centers and Community Mental Health Centers in the state that choose to participate. These two programs are called The Health Link and the Patient Centered Medical Home (PCMH) programs. Navigant is providing statewide training and technical assistance support for Medicaid provider practices that participate in TennCare and that are enrolled in these two innovative programs.
The State is enrolling providers in phases, with the first phases having started December 1, 2016 for The Health Link and January 1, 2017 for the PCMH program. This is a three-year engagement for which Navigant will provide various training support, including onsite coaching. Each participating provider site will be eligible to receive onsite coaching along with periodic learning collaboratives that include all of the participating organizations.
The **Tennessee Transformational Coach** is responsible for providing support to Community Mental Health Centers (CMHCs) and/or primary care practices to assist them in the development and transformation to a health home or PCMH model with capabilities to manage the physical and behavioral health needs of their assigned members. The coach will effectively train and equip practices with the best approaches to optimize and improve their patient's health outcomes, coordinate other physician health practitioners to employ a holistic approach to coordinated care while controlling costs and enhancing provider/patient interaction. He/she will support the program's quality performance objectives through utilization of established quality improvement methodologies, quality improvement tools and techniques, and team facilitation activities. **Further information about Tennessee's Primary Care Transformation is available at: -care-transformation** .
**The GHS team is currently looking to hire a coach/manager who resides in Tennessee; ideally in the Nashville area.** The coach/manager will have the following roles and responsibilities:
+ Based on initial assessments determine the level of need for ongoing training as well as to develop individualized coaching plans
+ Act as a liaison between the practice, Navigant leadership, and state personnel overseeing the program
+ Manage and oversee junior level Navigant staff
+ Assure quality work product of self and junior level Navigant staff
+ Assist with project management responsibilities
+ Conduct onsite training and coaching in the providers' offices to support providers with transformation to health homes or PCMH models.
+ Use approved curricula and NCQA PCMH recognition requirements (as necessary) to conduct coaching and address practice transformation issues
+ Act as a resource to link practices to additional information or support
+ Conduct semi-annual assessments at the practice level, but including site-specific information, to identify progress and to determine any changes in training needs
+ Help prepare for and participate in the following trainings conducted by Navigant for providers as required by the State: large conferences, learning collaboratives and webinars
+ Provide presentations during conference trainings or webinars and facilitate learning collaboratives based on expertise
+ Provide all documentation required by the Department for real-time tracking of practice activities
+ Five (5) or more years of previous work experience in the health care industry or with a consulting firm
+ Five (5) or more years of relevant experience in coaching/training behavioral and physical health integration
+ Experience working with both small and large provider practices
+ Knowledge of primary care transformation processes, PCMH transformation and health home delivery models
+ NCQA PCMH Content Expert Certification preferred
+ Demonstrate knowledge of quality improvement, patient engagement, behavioral health treatment, service delivery and care coordination
+ Possess the confidence and skills necessary to effectively train practices, and equip them with best practices and approaches to optimize their population health impact, control total cost of care and improve the patient experience
+ Possess the confidence and clinical expertise necessary to provide presentations during conference trainings or webinars and facilitate learning collaboratives

+ Practice workflow redesign and management

+ Enhanced patient access (e.g., flexible scheduling, expanded hours)
+ Evidence-informed and shared decision making
+ Developing an integrated care plan
+ Patient and family engagement (e.g., motivational interviewing)
+ Making meaningful use of HIT/ HIE
+ Performance measurement including quality measures
+ Clinic operations
+ Clinical workflow management
+ Frequently communicates with clients and coworkers and must be able to share information effectively
