In conjunction with the SCH Case Management System, the Renal Case Manager uses the patient focused care process (assess, plan, implement and evaluate) to facilitate implementation of a comprehensive, collaborative plan of care for adult medical/oncology/renal patients across a continuum. Utilizes effective, efficient and appropriate resources and monitors individual patient outcomes as well as trends. Facilitates development, implementation and evaluation of evidence-based nursing practice. Implements and abides by Customer Service Standards. Supports and implements patient safety and other safety practices as appropriate. Supports and demonstrates Family Centered Care principles when interacting with patients and their families and with coworkers. Develops practice consistent with Magnet Forces.
EDUCATION
Bachelors Degree from an accredited school of nursing required.
EXPERIENCE
Minimum of 2 years of clinical experience in medical nursing that includes renal populations; has critical thinking skills, has experience and skills with patient education, experience in working with teams, can demonstrate skills in communication, delegation, conflict resolution and other group processes and techniques. Knowledge/experience in discharge planning, CQI theory, financial/payer reimbursement.
SPECIAL CONSIDERATIONS
Current MN licensure as a Registered Nurse required.
Certification will be required as part of position.
