About Amedisys Home Health and Hospice

Our nurses, therapists and other caregivers deliver professional, personalized health care services to patients in the comfort of their own homes. “Taking care of your loved ones as we would our own” is the philosophy Amedisys was built on. This simple concept has guided Amedisys’ growth into one of the nation’s leading home health and hospice providers.

Amedisys is a recognized innovator, one of the first in the industry to equip its clinicians with point-of-care laptop technology and referring physicians with an internet portal that enables seamless, real-time coordination of patient care. We also have the industry’s first-ever nationwide Care Transitions program, designed to reduce unnecessary hospital readmissions through patient and caregiver health coaching and care coordination.

The health care industry is changing; at Amedisys we’re working to lead the country towards a new model of care that not only saves money, but provides convenience, compassion and customized service for each and every patient. We provide communication, education and health care for patients with post-acute care and chronic disease management needs. We are a partner in the continuum of care for hospitals and for physicians.
